Turn meeting notes into next steps
Open your notes after a conversation. Record what was actually decided, which action you accepted and what still needs clarification. A suggestion is not a commitment yet. This is your personal working note; the agreed meeting record and actual agreements remain the reference.

How to use the worksheet
Turn meeting notes into actions you can check
Start with the actual outcome
Read your notes and the agreed outcome document. Mark unclear points rather than recording a memory as a confirmed decision.
Make your own action executable
Write an accepted action with a visible first step. Another person’s responsibility or date is confirmed only when they have actually agreed to it.
Short version: decision, own action, open question
For a brief follow-up, fill in just these three areas. Leave unconfirmed details explicitly open. The worksheet does not create or send tasks.
Check dependencies and prepare a question
Note what is missing and whom to ask. Your own check time is not a deadline for someone else. Verify agreements directly and copy or print before closing.
A completed example
Example: start the outline and clarify the missing image separately
Fictional example for illustration. Adjust times, tasks and responsibilities to your circumstances.
- Conversation and note source
- Fictional team meeting on Tuesday about a presentation. Sources: my notes and the agreed outcome document.
- Decision or still a suggestion
- A short introduction was agreed. An extra overview was suggested but has not been agreed yet.
- Action I accepted
- I accepted the action of adding the introduction outline to the shared document. My first step is to open the document.
- Who confirmed what, and by when?
- I confirmed Thursday for my draft. Sam has not committed to providing the material, so I will not list Sam as a confirmed owner.
- What prerequisite is still missing?
- An approved image is missing for the last section. I can start the outline before it arrives and mark the image space as pending.
- Question, own check and short version
- Ask Sam about the image and a possible time; message not sent yet. Check for a reply on Wednesday without treating that as a new commitment. Short version: decision, own action, open question.
